Waiver of Liability and Payment Policy
By registering for this course I understand that yoga therapy includes diverse activities such as, but not limited to, physical activity, breath work, meditation, self- reflection and lifestyle guidance. As is the case of any physical activity, the risk of serious or disabling injury or death is present despite the best efforts of all involved.
I further understand that yoga therapy is not a substitute for medical attention, examination, diagnosis or treatment. I understand that before beginning yoga therapy, it is recommended that I consult with my physician to ensure that I am physically and psychologically ready to participate and assume all risks connected with yoga therapy. I acknowledge that yoga therapy is not a licensed healthcare profession.
I agree that if I experience any pain or discomfort -- whether physically or emotionally -- I will listen to myself and discuss such with my physician and/ or therapists. I will never do anything that causes pain. With a full understanding of the potential risks, I hereby assume the risks of yoga therapy. I affirm that I alone am responsible to decide whether or not to take the suggestions offered by the yoga therapist.
I hereby agree to irrevocably waive, release and discharge any claims and/or liabilities for personal injury or damages of any kind that I have now or hereafter may have against Divya Jyoti Yoga Therapy, Yoga Therapy Academy and Ana Mares Guia and/or any representative or employee associated with this course thereof as a result of my choice to participate in the Yoga Therapy Academy for the Healthcare Community.
In consideration of participating in any services provided by Ana Mares Guia and Divya Jyoti Yoga Therapy Center and Yoga Therapy Academy including but not limited to yoga and any wellness instruction and services, I hereby agree to the following:
1. I understand that the services provided by Ana Mares Guia, Divya Jyoti Yoga Therapy Center and Yoga Therapy Academy may be strenuous or cause physical injury. I am fully aware of the risks and hazards potentially involved and I am willing to assume those risks. I understand that when I participate in these services that I will also receive general health information. Any recommendations for changes in diet are entirely my responsibility and I will undertake to consult a physician prior to undergoing any dietary or food supplement changes.
2. I have been fully advised that no exercise/activity/yoga program should be undertaken without the consent of a medical doctor and I am responsible for obtaining such consent. I represent and warrant that I am not aware of any medical condition(s) that would prevent my full participation in this course.
3. I voluntarily and expressly, with knowledge waive any claim that I might have against Ana Mares Guia and Divya Jyoti Yoga Therapy Center and Yoga Therapy Academy for any risks, injuries and/or damages, known or unknown, which I may sustain as a result of my participation in this course.
4. I hereby forever release, discharge and waive any claim or rights that I may have against Ana Mares Guia, Divya Jyoti Yoga Therapy Center and Yoga Therapy Academy for any injuries and/or damages sustained due to their negligence or the negligence of others, and covenant not to file any suit against Ana Mares Guia, Divya Jyoti Yoga Therapy Center and Yoga Therapy Academy for any such injuries and/or damages. The parties hereto intend to Release and Waiver of Liability to be binding upon their heirs and any and all successors-in-interest.
5. I understand that the services provided by Ana Mares Guia, Divya Jyoti Yoga Therapy Center and Yoga Therapy Academy may be involve self-reflection and physical movement and I take full responsibility for knowing, monitoring and acting within my abilities and learning and incorporating any modifications or adaptations necessary to proceed with such activities in a safe and appropriate manner.
PAYMENT, REFUND & CANCELLATION POLICY
Payment for all services is required in full at the beginning of the course.All sales are final. All payments for services are non-refundable and non-transferable.
I have read and understand the above Release & Waiver of Liability, Payment, Refund, & Cancellation Policy; and its contents. I hereby voluntarily agree to the terms and conditions set forth herein.
